We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services.
We may use and disclose your PHI to bill and collect payment for services.
We may use your PHI for internal operations including quality improvement, staff training, and compliance activities.
We may contact you with reminders via phone, text, or email. We will never include clinical or health details in marketing messages.
Most uses beyond those above require written authorization, including use for marketing purposes, sale of PHI, and use of your photos in promotional materials. You may revoke authorization at any time in writing.
You may inspect and obtain a copy of your PHI within 30 days of written request.
You may request amendment of inaccurate or incomplete PHI. We will respond within 60 days.
We must honor your request to restrict disclosure if you have paid out-of-pocket in full for the services rendered.
You will be notified of any breach of your unsecured PHI within 60 days of discovery.
